If you want a runny egg, you'll also get a rare burger, so at least if you've ground the beef yourself, you know you're not getting lungs, stomach etc.
You're right but I'd be more worried about e-coli or listeria. Instead of grinding your own meat, you could cook the patty 3/4 of the way and add the egg near the end, giving it just enough time to cook to over-easy.
