The reason for this is that Nick and Bianca are actually both transgender. Nick was born a girl named Nicole and Bianca was born a boy named Jason and although they live as the opposite genders now, they still have their respective female and male parts as they have not been able to afford surgery to change their bodies completely. The couple have two children, Kai and Pax and Nick gave birth to both kids while Bianca provided the sperm!
