wanted to delete a DNS record. deleted the whole DNS server. for the largest Casino in the nation. had to shutdown 7 locations. cost 2 million euros. oops!

I work in the aviation industry, had a little accident that cost the company 32 million US dollars. I still get strange looks today and have a safety video dedicated to me, still work for the same company, 5 years after my little accident.
