#5 I gotta say, I thought that was intentional art. I was trying to figure out what "they were doing" for the fuel hatch since it looked different from what was around it.

It looks like my handwriting after I get annoyed by some saying they can't tell what It says, but you are right it is more than likely that a nurse wrote the note - my dentists rarely if ever write anything like prescription/instructions themselves, but get their nurse/aid to do that instead.

Dr's in my state don't usually write their own notes. The assisting nurse does it after he leaves the room.
