#22 Pretty sure this wasn't the fault of any government agency. It was either the phone company or the so-called IT/Communications person who worked on it before you got to it.
#11 IF the folded metal was a real CD than all data is recoverable. As long as the silver part is intact the info is intact, the plastic is just something to attach the silver data sticker to. I bet a forensic tec could do it.
