What Dreams May Come, with Robin Williams. I developed my first stutter after seeing it in the theater, and still have only watched it through three times... oh and Grave of the Fireflies... can vividly recall many scenes but only could watch it twice (second time to show my new husband).

As far as twist endings go, Primal Fear is a classic. That one got me good.
I was also fascinated with the Abyss the first time I saw it. The idea of breathing liquid freaked me out.

I have experience with addicts and addiction, and Requiem for a Dream is incredibly realistic. It was hard to watch, and I'll never do it again. Drugstore Cowboy, too.

I always thought The Truman Show was kinda dumb, but to each their own.

Requiem for a Dream is the movie I've introduced to all of my friends. It's such a realistic portrayl of deep drug addiction.
But, if you want movies that really blow you away try watching Canibal Holocaust, Gummo, and Kent Park. Warning, Kent Park is NOT a movie to watch with other people. It's is the MOST uncomfortable I have ever been watching a movie, even more so than the last 20 mins of Requiem. Oh, and Spun!
