Mindy You are right about the profile (45 in this case) being percentage of the tire width. R19 indicates the radius (i.e rim size if you will); 19 inches in this case.

V is indeed the speed rating (V=240km/h) on the depicted tyre. R is also a valid speed indicator, for maximum speed 170km/h. If R was the speed indicator it would be alone at the end like the V is here.
