My guess is you're supposed to bolt that down not just use duct tape

Bolt what down? That base plate looks like thin plastic with electronics in it.

if you look closely, the chair while rotating touched the metal stand (for keeping people away of the simulator) on the left, and by doing that made the whole thing turn. I'm guessing the machine is not at fault but human error.
