Other reasons is structural problems IE bad steel etc ...
I know a guy who reloaded some .40 ammo and used it in his Glock.. the slide shot back and impaled into his face.. 3-4 surgeries later you can hardly see the scars anymore.. He didn't make the ammo a "HOT" load on purpose he accidentally double charged that one round..
So pay attention people
