You know, most movies try NOT to have mistakes in them, which is why this shot from Gravity is so bizarre. If you look in the reflection of Kowalski’s helmet, you can see what appears to be a camera and boom mic reflected in the glass. So, it’s a goof, right?

Not exactly. None of the glass on the astronaut helmets in Gravity is ever real during its entire runtime, its always computer-generated. The reflection is also computer-generated. Director Alfonso Cuaron inserted it to make it look like the scene was shot in space with a camera crew, not in a studio. You know, for laughs.
