Fat meant you were rich enough to buy enough food. Just look at the paintings of the middle ages especially by Rubens.
It's the same with skin tone. Getting a tan back then meant you worked in the fields leading to pasty white being beautiful.
Being thin and tan now is the standard because it takes money and willpower especially now when anyone can buy candy.
