The shark landed with half its body inside the cage and began trying to take bites out of him. DiCaprio flattened to the bottom of the cage until he could be rescued.

Well, Lundgren hit him so hard that Stallone’s heart slammed against his breastbone and began to swell. Without medical attention, his heart would have swollen until it stopped.
